Suggestions on what to bring to your tax appointment:

  • Social Security Cards for yourself and each dependent.
  • W-2s from your employers
  • 1099-MISC forms for self-employment income
  • 1099-INT (interest) and 1099-DIV (dividends) forms
  • 1099-B forms showing brokerage trades in stocks and bonds
  • 1095-A marketplace Health Insurance statement
  • 1098-T tuition and scholarship statement for higher education
  • K-1 forms for income from a partnership, small business, or trust
  • 1099-SSA form showing Social Security received
  • Written documents for additional income not reported on a W-2 or 1099 form, such as other self-employment income, or rental income.
  • 1098 forms reporting mortgage interest
  • 1098-E forms reporting student loan interest
  • Copies of the previous years tax return (If you are a first year client)
  • Cancelled checks, bank statements or receipts along with summary of any estimated federal and state payments made in the year of the tax return
  • Cancelled checks, bank statements or receipts from any estimated taxes paid to your city.
